A new plan to fix problems with improperly cleaned surgical instruments at the Detroit Medical Center has been approved.

It came after the state opened an investigation following the Detroit News a story in August documenting years of doctor complaints.

The DMC announced plans that include the formation of a surgical improvement council and task force to oversee instrument sterilization. We're told the plan also better addresses better policies for cleaning.

The medical center admitted that unclean surgical tools forced doctors to delay procedures and keep patients under anesthesia unnecessarily.

According to the DMC, no one was ever harmed.
